What should I bring to my appointment?
- Photo ID
- Insurance card(s)
- List of prescription meds
- Completed Patient Forms - Download here (forms also available in office)
- Co-payment
- Pertinent labs and x-ray studies
- Pertinent outside medical records
What are acceptable forms of ID?
We accept all government issued photo ID’s. This includes drivers’ license, identification cards, passports, and or resident alien cards (regardless of state or country of origin)
I don’t have a copy of my most recent labs and or diagnostics, what can I do?
If this information is not available at the time of your visit, please ask a member of our front desk team for a medical release form. Once completed we can submit the request to the facility and or doctors office to have that information submitted to us. Having this information is imperative to developing a plan of care for your condition?
How much will I have to pay at the time of my appointment?
Patient responsibility is determined by your insurance company. Once benefits have been verified, an associate will call you to advise of any patient responsibility due at the time of visit. We kindly ask that you come prepared to pay any copays, co-insurances and or deductibles assigned.
I have an outstanding balance but can’t afford to pay it in full, can I make payment arrangements?
Payment arrangements can be negotiated for MOST outstanding balances. The terms of said arrangements will depend on the amount of the balance and the age of the account. Please feel free to contact a member of our billing staff for more information or for additional assistance?
How long will I have to wait?
Wait times vary based on patient volume, complexity of the patients being seen and the number of physicians in the office on any given day. Rest assured we value your time and continually strive to make your appointment as smooth and efficient as possible.
What if I need to talk to someone outside of normal business hours?
Our physicians are available after hours for all urgent issues. For all non-urgent related issues such as prescription refill requests, lab or diagnostic results or billing questions, kindly call us back during normal business hours. In case of emergency, please call 911 or visit your nearest emergency room.
What hospitals are the doctors affiliated with?
Our physicians have privileges at Memorial Hermann Katy, and Methodist West
I don’t see my health insurance plan listed on your accepted plans, does that mean you don’t accept my insurance?
Absolutely not. We accept a wide variety of health plans. Please feel free to contact a member of our staff to inquire which plans are accepted.
